Health Services

Nurses are on staff and available in all Waynesville school buildings to assess, plan, and implement coordinated school health services.  Our district has worked toward integrating health education and health-related services into the everyday school experience. 

Children need to be healthy to learn, and we want to educate students to be healthy.  Our program is designed to promote the health of students, identify and prevent health problems and injuries, and ensure care for students.

School Health Services:

  • Assess the health and developmental status of all students.

  • Identify students with special health concerns and develop health care plans with students and families.

  • Establish and maintain comprehensive school health records.

  • Establish and maintain immunization records that comply with state laws, rules and regulations regarding immunization requirements.

  • Conduct annual screening programs tailored to each age level, make referrals, and provide follow-up as needed.

  • Monitor communicable disease prevention and control programs.

This letter is to inform you of the health policies regulated by the Waynesville R-VI School District.

Missouri Immunization Requirements
It is the policy of the Waynesville R-VI School District that all students attending the district schools shall be immunized in accordance with law.  The district will not allow a student to attend school until the district has satisfactory evidence on file that the student has been immunized, that the immunization process has begun and satisfactory progress is being accomplished or that the student is exempted from obtaining immunizations in accordance with law.

Immunization Exemption Forms

Religious Exemption Form --  Claiming a religious exemption represents a parent or guardian’s belief that the family’s religious preference does not support immunizing against vaccine-preventable diseases.  A religious exemption can be filed for selected vaccines or for all vaccines. Parents and guardians should indicate which vaccines are included on the Religious Immunization Exemption. The exemption can be obtained from the local health department and must be completed and filed with the school nurse.

Medical Exemption Form -- Claiming a medical exemption represents a physician’s determination that the child is allergic to some
immunization components, has an immune deficiency or has an illness such as cancer.  A medical exemption can be obtained from the local health department or physician. The medical exemption must be signed by a physician and filed with the school nurse.

Immunizations in Progress Form -- Unfortunately, some children fall behind in getting their age-specific immunizations required for child care and school attendance.  According to Missouri regulation, children who have not received immunizations required for child care and school attendance cannot attend until their immunizations are up-to-date.  However, a child is allowed to attend if the required immunization series has begun and an appointment for the next dose is scheduled.  An in progress exemption can be obtained from the local health department or a physician.  The in progress card must be signed by a physician, public health nurse or designee and filed with the school nurse.

Does your middle school or high school age child wish to participate in school sports?

MSHAA Physical Form (Sports - WMDS & WHS only)

This form must be completed annually by a physician and on file with the Athletic Office (WHS) or Nurse's Office (WMDS).  The physical must be completed on or after February 1 prior to the ensuing school year.

Does your child require prescription medications at school?

Permission for Non-Prescribed Medicine

This form must be signed by the parent for over the counter medications before any medication can be given by the school.  A medical authorization form must be on file in the nurse’s office if medications are to be dispensed.  A new form must be presented to the school each new school year. Medications must be in the original container. Students are not allowed to transport medications except when authorized by a physician.
